Welcome to your new Medicare Advantage plan

Thank you for choosing Summit Health, where you have access to local providers you already know and trust. We’re excited to support you on your path to better health and wellness. To begin, let’s start with the steps below.

Schedule your annual wellness visits

If you are within the first 12 months of signing up for Medicare, make sure you get the Welcome to Medicare preventive visit from your primary care provider. Your provider will use this visit to develop your personalized prevention plan. This plan will be based on your current health and risk factors. This visit is yours at no cost. If you have been a Medicare member for more than 12 months, you can get a Medicare yearly wellness visit. You can get the Medicare yearly Wellness visit once every 12 months for no cost. Please note that you cannot get your first yearly wellness visit within 12 months of your Welcome to Medicare preventive visit.

Talk with your primary care provider about preventive services and screenings. Vaccines are also important. Ask your provider what vaccines you need. And, be sure to review your prescription and over-the-counter medications, vitamins and supplements with all of your healthcare providers at each visit. Pay your premium

There are several ways you can pay your monthly premium. See your options. How to pay

Your network

Use our Find Care tool to search our Medicare Advantage network of healthcare providers and hospitals. Please know that not all providers are in our network. Make sure you choose providers who are in our network. This will save you money. To choose an in-network provider, you can use Find Care. Please be aware that our provider networks may change. If this happens, we will let you know. Please note: If you are having a medical emergency, you do not need to see a Medicare provider. Instead, call 911 right away, or go to the nearest emergency room. You do not need to get approval or a referral from your primary care provider. A medical emergency is anything that is life-threatening. Examples include chest pain, uncontrolled bleeding and difficulty breathing.

Know your benefits

Get the most out of your plan. See what's covered and not covered. Make sure to visit your Member Dashboard, and review your Evidence of Coverage (EOC). Your EOC is your member handbook. To find out what medicines are covered, review the formulary (list of medication).

Know your no-cost benefits

As a Medicare member, you have many preventive services and screenings available for no cost. Preventive care is necessary for good health. Examples of your no-cost benefits are:

  • Blood pressure
  • Breast cancer screening
  • Colon cancer screening
  • Diabetes
  • Depression
  • Sexually transmitted infections (STIs) screening and counseling

Know your supplemental benefits

As a Summit Health Medicare member, you have the following supplemental benefits included in your plan. Login to your Member Dashboard for benefit flyers and more plan information.:

  • Quarterly funds for over-the-counter (OTC) health items and services to purchase at participating online and retail stores.
  • Routine vision exam and eyewear benefits through VSP Advantage
  • Preventive and comprehensive dental benefits
  • Routine hearing aid exam and hearing aid coverage provided through TruHearing
  • Gym membership and wellness resources from Silver&Fit®
  • Registered Nurse Advice Line, available 24/7. Call 866-321-7580. TTY users, please call 711.
  • Coverage for routine chiropractic, naturopathic, and acupuncture services embedded in your plan at no extra premium.
