Mailed Screening Kits for Your Patients

Summit Health is partnering with BioIQ, a leader in at-home healthcare solutions, to mail eligible Summit Health members a fecal immunochemical test (FIT) and/or a hemoglobin A1c at-home test kits in September.

Members are eligible for this outreach if they are noncompliant for the Colorectal Cancer Screening and/or the Diabetes Care – HbA1c Controlled Star Measures for Measurement Year 2022. These kits are at no cost to the member, if their membership is still active when the kit is received for processing.

The FIT and A1c kits can be completed at the convenience of their home and returned to BioIQ using an enclosed prepaid and pre-address return envelope. Lab results are mailed to the member and their attributed primary care provider (PCP) on record. If a test result is outside of the normal range, BioIQ will conduct live telephonic outreach to the member to discuss the results and recommend follow-up with their PCP.

We ask that you encourage your patients to complete these kits if received, and we hope that this is an impactful way to help keep your Summit Health patients up-to-date with their preventive services and connect them back to your clinic office for any needed follow-up. September is Healthy Aging Month

Healthy People 2030 identified “Improving the health and well-being for older adults” as one of the ten public health priorities in the U.S. The focus for Healthy Aging Month is to bring awareness to the health and well-being of older adults and supporting them to stay healthy and independent.

Summit Health knows how important it is for our members to see their PCPs, stay active, eat healthfully, and take precautions to reduce risk of falls in their homes. This is why we encourage members to get their Annual Wellness Visits (AWV) include access to gym memberships as a supplemental benefit and support our members with health coaching and other programs.

Encourage your Summit Health members to complete their AWV today. Your Summit Health patients have a covered AWV (once every calendar year) and an Annual Physical Exam (once every calendar year) at no cost. These can be completed at the same visit and can also be combined with a problem visit.

A cost-share would apply if an AWV or Annual Physical Exam is combined with a problem visit. Be sure to explain any additional services that may be done during a visit so that your patients understand their expected out-of-pocket costs.

Summit Health Fall Clinic Trainings

Our EOCCO and Summit Health Quality Improvement (QI) team will be conducting clinic trainings this Fall to review year-to-date clinic performance against MAPCIP incentive measures and discuss year-end strategies. These will be held in-person or virtual, depending on your clinic’s preference.
