We are excited to announce that the 2023 instance of the Care Gap Management Application (CGMA) by Novillus is now live and ready for you to onboard. This is a web-based tool that allows clinics to view and close care gaps, capture Hierarchical Condition Codes (HCC’s) throughout the year, manage your patient roster, and view your incentive program progress.
The CGMA onboarding process does not require any lift from clinics to get started, besides simply setting up login access. If you had access to the 2022 instance, your same credentials can be used to access the updated instance.

We are committed to working with you to bring better health and overall wellness to your patients by conducting outreaches throughout the year to your patients and your clinic. These programs aim to encourage your patients to close recommended preventive services, follow-up with those who recently discharged from inpatient stays, and provide support for high-risk patients.
To keep you informed of the member- and provider- outreach programs that Summit Health conducts, access our 2023 Summit Health Provider Quality Initiatives Calendar.
Our EOCCO and Summit Health Quality Improvement (QI) team will be conducting clinic trainings on the 2023 Medicare Advantage Primary Care Incentive Program (MAPCIP) and quality measures. These will be held in-person or virtual, depending on your clinic’s preference.
