Behavioral Health Authorizations

Some Behavioral Health services require prior authorization. Please review our list of treatment services that require prior authorization. To request prior authorization please call Summit Behavioral Health (833) 460-0445 or submit the Medicare behavioral health authorization form.

Clinical guidelines

Outpatient psychotherapy

We recommend consistent use of outcomes measures in outpatient psychotherapy.

Substance use

We encourage all providers to screen for possible substance use problems using a validated tool such as the AUDIT or SBIRT screening.

Depression

We encourage all providers to screen for possible depression problems using a validated tool such as the PHQ-9 or Ham-D. PHQ-9 scoring instructions.
