Please let us know if there are ways that we can serve you and your Summit Health patients better.
Summit Health is enthusiastic to share information regarding our streamlined Provider Data Exchange (PDE), an automated incentive-earning data share program to help providers close care gaps. We are flexible and accommodating to support any lift required to set up the seamless supplemental and clinical data sharing process.
Summit Health builds a Secure File Transfer Protocol (SFTP) Site, data schemas to integrate your data into our internal data warehouse, and valuable reporting on measurement gap lists and incentive earnings that are shared through monthly provider reports accessible on our Provider Reports Portal.
You can then submit data using a secure File Transfer Protocol (FTP) client. Data can be submitted in up to four categories (diagnostics, labs, vitals, and screenings and assessments) monthly or other regular cadence through your dedicated SFTP site. Not all four categories of data are required, but highly recommended to impact potential incentive earnings.
Once initial data is submitted, we will conduct an audit for quality and ensure all schemas are integrating your data appropriately. After validation, the process will be automated to intake your data and produce updated accurate monthly reports.
