In-Home Assessments to Support Your Patients

In-home Assessments for Your Patients

Summit Health has partnered with Signify Health to offer no cost In-home Health Assessments (IHA) to select Summit Health members during the 2022 calendar year beginning February 21, 2022. Members chosen to participate will be offered the opportunity to have either an In-home or Virtual health assessment completed by a licensed Signify Health practitioner from the comfort of their own home.

The In-home and Virtual assessments generally include:

  • A comprehensive exam
  • Assessment of clinical and personal history
  • Medication reconciliation
  • Assessment of risk factors including social determinants of health and falls risk, if completed In-Home
  • Appropriate health education and guidance based on completed assessment

This service is again offered at no cost to our members, does not use any member benefits, and will not replace the Annual Wellness Visit (AWV). The IHA is intended to help our members, their PCP’s, and Summit Health to better understand and manage healthcare needs and case management opportunities.

If your patients are eligible and complete an IHA, you will receive an after visit summary letter directly from Signify Health. Your patient’s summary letter will include comprehensive assessment outcomes and member health recommendations to assist in making informed decisions about your patients health.

March is National Colorectal Cancer Awareness Month

Colorectal cancer is the second leading cause of cancer-related deaths for men and women in the U.S.1 Screenings can detect precancerous growths at an early stage, when treatment has the highest likelihood of success.2

We encourage you to use the outreach rosters on the Summit Health provider progress reports to identify your patients that are due for a colorectal cancer screening. Colorectal Cancer Screening is a Medicare Star Measure defined as the percent of members ages 50 to 75 who had appropriate screening for colon cancer.

Our Summit members 50 and older have no cost-sharing 3 when seeing in-network providers for the following:

  • Flexible sigmoidoscopy (or screening barium enema as an alternative) every 48 months
  • One of the following every 12 months: Guaiac-based fecal occult blood test (gFOBT) or Fecal immunochemical test (FIT)
  • DNA based colorectal screening every 3 years
  • For people at high risk of colorectal cancer, we cover a screening colonoscopy (or screening barium enema as an alternative) every 24 months
  • For people not at high risk of colorectal cancer, we cover a screening colonoscopy every 10 years (120 months), but not within 48 months of a screening sigmoidoscopy

Ensure patients are provided with all their colorectal cancer screening options and discuss what is most appropriate for each patient. Access more resources and information about colorectal cancer at the American Cancer Society.

Upcoming Events & Training

Summit Health Spring Clinic Trainings

We are currently in the process of finalizing our 2022 Summit Health value-based contracting, which we expect to be mailed to you by late-March.

In support, our EOCCO and Summit Health Quality Improvement (QI) team will be conducting clinic trainings in April on the 2022 VBC and quality measures. These will be held in-person or virtual, depending on your clinic’s preference.
