Upcoming Patient Experience Survey

The Centers for Medicare and Medicaid Services (CMS) has made member and patient experience a top priority with Consumer Assessment of Healthcare Providers and Systems (CAHPS) survey measures now making up 33% of the Star Ratings calculations for Medicare Advantage. These Star Measures include rating the experience with the health and drug plan, health care quality, access to care, and care coordination between providers.

Summit Health is committed to providing great member experience and continual improvement. One of the ways we are doing this is by partnering with Symphony Performance Health (SPH) to conduct a mock CAHPS survey with a sample of our Summit Health members in August. This mock CAHPS survey will help us collect member feedback and use the data to inform improvements to the overall member experience.

We ask that you support us in encouraging your patients to respond to the survey if they are selected in the sample. This way, we can share de-identified aggregate feedback to you at the end of this outreach. Many of your patients may receive the 2023 Medicare CAHPS, and the interactions you have with your patients and their perception will directly impact CAHPS Star Measure Ratings.

 

Support for Year-Round Data Capture and Care Gap Closure

In support of our Summit Health Medicare Advantage Primary Care Incentive Program (MAPCIP), we are implementing a web-based Care Gap Management Application (CGMA) by Novillus this Summer. This tool allows clinics to view and close care gaps, capture Hierarchical Condition Codes (HCC’s) throughout the year, manage your patient roster, and view your incentive program progress.

The CGMA will provide:

  • Capability for clinic staff to easily upload charts year-round for HCC capture and care gap closure for both Medicare Star Measures and risk adjustment, alleviating administrative burden at the end of the year for chart requests
  • Ability to track and manage quality performance and incentive earnings on the Medicare Advantage Primary Care Incentive Program (MAPCIP)
  • Ongoing provider coding and documentation education by Novillus

Our goal in implementing the CGMA is to provide a valuable and easy-to-use tool for our clinic partners to capture HCCs and close care gaps.

We understand that many clinics may already be utilizing this CGMA or sharing data in other ways. Our implementation strategy will be tailored to your clinic’s needs. More to come in the next few weeks!

 

August is National Immunization Awareness Month

Vaccines are safe and effective at preventing serious disease. As trusted providers of the Summit Health community, you play a key role in educating patients about the importance of adult vaccinations.

The Centers for Disease Control and Prevention (CDC) reports that adult vaccination rates are extremely low because most adults are not aware that they need vaccines.

  • Assess immunization status of your patients at every visit
  • Strongly recommend vaccines they may be due for
  • Speak with patients that may be hesitant to answer questions about vaccine safety, risk, or other concerns to reassure them that vaccines are safe and effective at preventing serious diseases
  • Administer recommended vaccines or refer to a vaccination provider

Encourage your patients to schedule appointments to ensure they are up to date on annual exams and recommended vaccines. Your Summit Health patients have a covered Annual Wellness Visit (once every calendar year), an Annual Physical Exam (once every calendar year), and certain vaccines covered at no cost including pneumonia, influenza, Hepatitis B vaccine, COVID-19, and other vaccines if they are at risk and meet Medicare Part B coverage rues.

Visit the CDC for the Adult Immunization Schedule and other educational resources and Continuing Medical Education (CMEs) for healthcare professionals.

 

Upcoming Events & Training

Our EOCCO and Summit Health Quality Improvement (QI) team will be conducting clinic trainings this Fall to review year-to-date clinic performance against MAPCIP incentive measures and discuss year-end strategies. These will be held in-person or virtual, depending on your clinic’s preference.
